This article is about patients with CHF and preventing readmission. It was found that patient education regarding CHF and their ability to perform self care have an impact on how well they do after discharge. Patient education programs that focused on self-care management had lower exacerbations symptoms and fewer readmissions. Patient's should be taught to weigh themselves daily, adhere to strict medication regimen and low-salt diet, self-monitor symptoms, exercise, and follow up regularly with their physicians.
